The Clinical Care Coordinator is a member of the Health Home team who, in collaboration with Nurse Care Coordinators and a Primary Care Liaison, works to integrate mental and physical health care for adults with serious mental illness and significant health concerns. Care Coordinators each serve 50 consumers and collaborate with internal and external treatment team members, identify resources, and reduce barriers for consumers to improve mental and physical health functioning. Primary responsibilities include:
assisting consumers in acquiring skills and resources necessary for successful community living that promotes identified physical and mental health outcomes.
provide support, consultation, and education to members of the consumers' network including family.
Participate in coordination of care activities to link consumer to appropriate community resources.
Provide transition of care support during hospitalization or emergency room visit in collaboration with Health Home team
Required Qualifications: Bachelors degree in social work, psychology or relevant human services discipline. Candidates must have a reliable vehicle as most of the work is done in the community.
