Community Connections is seeking case managers for a new collaborative project with CSOSA serving people currently under criminal justice supervision and living in the community. Case managers will coordinate offender services (medical, mental health, housing and vocational), provide 1:1 case planning, preparing treatment, aftercare and discharge planning. Energetic, mission driven individuals who want to make a substantial contribution to a new team and a new program.
Minimum qualifications for the Clinical Supervisor include:
BA degree and
Minimum of two years of experience in behavioral health and/or group home service settings
Successful completion of the Government’s background investigation is a condition of employment.
Candidates must have a reliable vehicle and clean driving record as most of the work is done in the community and may involve transporting clients.
