Community Support
The Community Support Program at Community Connections operates
on a modified case management model that includes a consumer-focused
empowerment stance. The consumer and his/her needs are the center
of the planning process. The case manager serves as a consultant
and a guide to support the consumer in the achievement of individual
goals. The relationship of trust and collaboration between the case
manager and consumer becomes the vehicle for assisting the consumer
in accomplishing goals. Additionally, the consumer and clinician
work together to create and enhance the natural support network
with the ultimate goal of having most of the consumer's needs met
by members of an ever-expanding community network. Consumers choose
to enroll in the Community Connections Community Support Program
and have a say about whom they work with on any one of five clinical
teams.
Services are available 24 hours per day, seven days per week and
often the clinical team provides help in the individual's natural
environment. A core goal of the CSP is to provide services that
enable the consumer to gain the best possible level of functioning
as defined by the individual.
