Clinical Services
Day Services
The Community Connections Rehabilitation Program is a facility-based
structured clinical program that includes cognitve-behavioral group
interventions and diagnostic, psychiatric, rehabilitative, psychosocial,
and counseling treatment. Three tracks are offered – a trauma track,
a dual diagnosis track and a skill-building track. Underlying all
of the services is the empowerment approach; consumers are encouraged
to choose the track that best suits their individual needs. Each
track has a fully manualized intervention with goals and outcomes
established to measure each individual's progress toward successful
community living.
The program operates five days per week Consumers can participate
all day, or they may choose to attend for a half day, three-hour
session. Lunch is served daily and other recreational activities
are planned. For more information, please contact the three program
directors, CMitchell@ccdc1.org,
