Programs for Youth
Since 1989 Community Connections has offered services to youth
who are in transition from educational residential care to supported
independence. The Youth in Transition Program is located in a seven
unit apartment building. Six of the two bedroom units house twelve
young adults ages 18-22. The seventh unit is a staff office with
24 hour staff supervision available. These young adults are encouraged
to make life choices and to understand the consequences of their
actions. At the end of their stay at Youth in Transition, most of
these youth move to greater independence. The Program has had real
success helping young people with severe emotional disabilities
to live and work in independent settings.
Small Group Homes: The small group home program, under the auspices
of the state of Maryland foster care program, accepts adolescents
ages 16 – 20 who are aging out of child and youth services and who
are returning to their home communities after a stay in residential
care. Each home is staffed 24 hours per day and has no more than
five adolescents. This home like atmosphere and attention to individual
needs has helped these high risk youth to regain a sense of mastery
and confidence in their environment. They attend school, assume
part-time jobs, and become involved in activities in the community.
Many of these adolescents have had histories of trauma and abuse
and have not had a safe and consistent place to call home. For those
who are ready, they can step up to a greater level of independence
through the Youth in Transition Program or to their own apartments
with support from staff.
