Continuum of Care
Community Connections offers a spectrum of service-enriched housing
options. The settings range from transitional and permanent staffed
group homes to specialized treatment houses to supported apartment
living. Though each housing component has unique features, the housing
network works as a total integrated system. The housing approach
at Community Connections tailors supports to meet individual's needs
within any given housing setting. Some of the housing is essentially
ordinary housing, and some of the housing is quite specialized and
really functions as an extension of treatment. Our goal is to achieve
the best possible fit over time, to support each individual's own
rehabilitation and recovery goals, and to be responsive to individual
preferences to the maximum extent possible without compromising
safety.
