Chad Holm, LPC

Director of Assertive Community Treatment and On-Call Services

Chad Holm, LPC is the Director of Assertive Community Treatment (ACT) at Community Connections and leads the agency’s On-Call Team for after-hour clinical services.  ACT is the agency’s largest clinical program, with seven teams and approximately 75 employees, and provides the most intensive level of community based mental health services to over 700 adult and young adult consumers.  Chad received his BA and MA in Psychology/Counseling Program from California State University Sacramento and is a Licensed Professional Counselor in Washington, DC.
