Clinical Case Management and Mental Health Worker - Rehabilitation Specialist on ACT Team

Intensive Community Mental Health Work:  Community Connections is the largest not for profit behavioral health agency in DC, serving vulnerable individuals, families, and children since 1984. Assertive Community Treatment (ACT) clinical staff, mental health workers and social workers serve clients collaboratively on multi-disciplinary teams serving those who are extremely vulnerable. ACT has general teams serving adults with mental health problems and specialty teams working with particular populations of focus: geriatric/traumatic brain injury, justice-involved adults, adults with both mental illness and substance use disorders; transitional age youth 18-29, and mothers with young children.  Social Workers are highly encouraged to apply.

Mental Health Worker Responsibilities:

  • Work collaboratively with team, completing daily rounds
  • Manage a small caseload of adult consumers
  • Provide community-based services focused on symptom management, overcoming trauma, and accessing resources
  • Reports to team supervisor, who is an LICSW or LPC    

Mental Health Worker Qualifications:

  • Requires at least an Associate’s Degree with preference for BA/BS degree or higher
  • Highly prefer (and greater responsibilities for) MSW or MA in counseling with DC LGSW or LGPC
  • Innovative, inspired, and energetic personality
  • Appreciation for street outreach and office-based clinical interventions
  • Excellent collaboration and communication skills
  • Pass Community Connections’ agency background check
  • Have a personal car and clean driving record
  • Be comfortable transporting clients in your vehicle
